The software works with health care providers' current electronic health record technology to lower patient hospital readmissions. By automating population health management, it boosts patient outreach and engagement.

"By combining our powerful population health management capabilities with the cognitive insight of IBM, once the acquisition closes, together we will help providers turn insight into action to improve health quality," said Steve Schelhammer, CEO of Phytel.

The software was just named the leader in the population health management category by the 2014 Best in KLAS Awards: Software & Services.

The other anticipated acquisition, Explorys, spun off from the Cleveland Clinic in 2009. It provides a secure cloud-computing platform that is used by 26 major integrated health care systems to identify patterns in diseases, treatments and outcomes. It integrates more than 315 billion clinical, financial, and operational data elements, spanning 50 million unique patients, 360 hospitals, and more than 317,000 providers.

Market intelligence firm IDC just named Explorys global leader in Healthcare Clinical and Financial Analysis.
